To obtain a copy of a police report:

Fill out this Police Public Records Request Form and email it to records@bullheadcityaz.gov, or fax it to (928) 763-5558.

Call 928-763-9200 to also request a police report over the phone. 

It can take up to 14 days from the date of the request before the report is available. 

You must request reports in writing, over the phone/fax, through an individual authorized by written power of attorney or through a licensed attorney at law who represents the victim.

Please provide as much of the following as possible:

1. Case Number
2. Names of persons involved
3. Date of occurrence
4. Location of event 
5. Your involvement; if any

If you have called ahead, please write down the name of the individual you talked to.

Victims receive one copy at NO charge. You must be listed as a victim on the report in order to receive the report at no charge.

When requesting reports in writing, please provide a self-addressed, stamped 9 x 12 envelope to have the report mailed to you.
